Key Responsibilities:
- Provide comprehensive social work services to hospice patients and their families.
- Deliver care based on physician orders and individualized care plans.
- Work under the direction and supervision of the Clinical Manager.
- Support patients with emotional, social, and community resource needs.
- Collaborate with interdisciplinary team members to ensure high-quality end-of-life care.
Qualifications:
- Master’s degree in Social Work (MSW) from a CSWE-accredited program.
- Minimum of one year of experience in a healthcare setting.
- Current licensure in the State of Michigan.
- Ability to pass criminal background and MVR checks.
- Completed health statements confirming the ability to perform job duties.
- Valid driver’s license.
- Proof of current automobile insurance.
- Proof of a negative TB test.
